This appears to be the book of prayers and order for a public fast printed and used in 1563. A book of thanksgiving was issued some time in 1564 to mark the end of the plague epidemic, however it is uncertain exactly when this occurred. See Mears et al, National Prayers: Special Worship Since the Reformation, vol 1 (Woodbridge: Boydell Press, 2013), 79-81.
